What is Complete Care PACE Program?
Complete Care PACE Program offers comprehensive healthcare services, including primary care, specialist visits, prescription medications, transportation, and more. All services are coordinated by our dedicated care team to ensure you receive the best care possible.
Our services also include adult day care, home care, physical therapy, and transportation to medical appointments and our PACE center in Van Nuys.
PACE Program is meant for senior adults over 55 years old who suffer from chronic conditions and require a higher level of care, but are able to remain in their own home.
Participants of the PACE Program receive care and benefits either in home, at the PACE center, or at a specialists office.
How much does PACE cost?
What you pay for PACE depends on your eligibility under Medicare and Medicaid.
If you qualify for Medicaid, you will not have a monthly premium for Complete Care PACE Program. If you are eligible for Medicare but not Medicaid, a monthly premium will apply.
There’s no deductible or copayment for any drug, service, or care prescribed or approved by our health care team
Is Preventative Care included?
Yes! Any service you would qualify for under Medicaid would also be provided under our PACE healthcare plan. This includes preventative care as well as chronic conditions care.
How can I qualify for Complete Care PACE program?
To be eligible for Complete Care PACE Program, you must meet the following criteria:
- Be at least 55 years old.
- Live in the designated service area (Van Nuys).
- Require a nursing home level of care, as certified by the state.
- Be able to live safely in the community with the support of PACE services
What about my previous doctors?
When you become a participant of our PACE program, all of your healthcare needs must be provided by our team or one of our authorized providers or specialist. This ensures that all services are coordinated for your well-being.
Our expert doctors and nurses will make the best care decisions based on necessity. Medical care is available around the clock, 7 days a week.
Do you provide transportation?
Yes! Free transportation is provided to our PACE center (for daycare activities) and for doctor/medical appointments.
How can I appoint a representative?
If you would like someone to act on your behalf regarding your care at Complete Care PACE Program, please fill out the CMS Appointment of Representative Form (CMS Form-1696). You can view and save a copy of the form here.
How can I access your PACE Program information in other formats and languages?
Complete Care PACE Program will provide, upon request, any marketing materials printed in English and other languages spoken by our community. Interpreter services are also available upon request.

How can I disenroll from PACE program?
You are able to disenroll from our PACE program at any time, no reason required.

What if I have a grievance or concern?
If you have concerns about your care or services at Complete Care PACE Program, you can file a grievance by calling us at (888) 605-0108 extension 930, sending an email to enroll@completecarepace.org, or submitting a written request to 6609 Van Nuys Blvd. #102, Van Nuys, CA 91405.
Participants also have the right to appeal any decisions regarding their care. Contact us at (888) 605-0108 extension 930 for more information on how to submit an appeal.
Participant Rights and Responsibilities Upon Disenrollment
- A participant may disenroll at any time for any reason.
- Voluntary disenrollment from Complete Care PACE program will be executed and effective on the 1st of the next month.
- Please provide as much advance notice as possible should you decide to disenroll.
- Upon disenrollment from Complete Care PACE Program, participants have the right to transition their care to other providers. Our team will assist with this process to ensure continuity of care.
- You must use our care services while your enrolled in our PACE program.
- Complete Care PACE Program will not be liable for any unapproved services provided outside our network, with the exception of emergency services.
To be eligible for Complete Care PACE Program, you must meet the following criteria:

- Live in the designated service area (Los Angeles County).
- Require a nursing home level of care, as certified by the state.
- Be able to live safely in the community with the support of PACE services
To enroll in Complete Care PACE Program, contact us to schedule an in-home assessment with one of our registered nurses or enrollment specialists.
This assessment will help us understand your health needs and confirm your eligibility.
When you join our PACE program, you will receive comprehensive healthcare services either at our PACE center in Van Nuys or in the comfort of your own home.
