Enrollment for Complete Care PACE Program
Enrolling in Complete Care PACE Program means receiving comprehensive care. The enrollment process begins with an in-home visit and a clinical assessment. During this visit, a Registered Nurse and Social Worker will discuss your health needs and determine your eligibility.
To be eligible for Complete Care PACE Program, you must meet the following criteria:
- Be at least 55 years old.
- Live in the designated service area (Los Angeles County).
- Require a nursing home level of care, as certified by the state.
- Be able to live safely in the community with the support of PACE services
To enroll in Complete Care PACE Program, contact us to schedule an in-home assessment with one of our registered nurses or enrollment specialists.
This assessment will help us understand your health needs and confirm your eligibility.
When you join our PACE program, you will receive comprehensive healthcare services either at our PACE center in Van Nuys or in the comfort of your own home.
What you pay depends on your Medicare and Medicaid eligibility:
-If you qualify for both Medicare and Medicaid (dual eligible), there is no monthly premium.
-If you have Medicare but not Medicaid, a monthly premium will apply.
-There are no deductibles or copayments for any services, drugs, or care approved by our health care team.
You may disenroll from Complete Care PACE Program at any time, for any reason.
